This notice is formulated in accordance with the Privacy Rule of the Health Insurance Portability and Accountability Act (HIPAA). It aims to inform you about how we collect, use, and disclose your Protected Health Information (PHI), as well as the rights you have regarding such information. PHI refers to individually identifiable information related to your health status, medical services, or payments, regardless of its form (paper, electronic, or oral).
We may use or disclose your PHI without your additional authorization for the following purposes:
PHI may be used or disclosed to provide, coordinate, or manage your medical care. For example, using your treatment records in internal quality audits to evaluate service effectiveness; or providing necessary information to medical regulatory authorities as required by law.
We may legally disclose PHI without your authorization in the following situations:
• Public health needs (e.g., reporting infectious diseases to health departments);
• Protecting your or others' health and safety in emergency situations;
• Responding to legitimate legal requests such as court subpoenas or search warrants;
• Child protection (e.g., reporting to relevant agencies when child abuse or neglect is suspected).
Under HIPAA, you have the following rights regarding your PHI:
You have the right to inspect and obtain copies of your PHI (including medical records, test reports, etc.). To exercise this right, please submit a written application to our Privacy Officer. We will respond within 30 days (in special cases, it may be extended by 30 days, and we will notify you in advance).
If you believe your PHI contains errors or is incomplete, you may submit a written application to request an amendment. We will review and inform you of the result within 60 days; if we refuse to amend, we will explain the reasons, and you may add a statement of disagreement to the PHI.
You have the right to obtain a record of non-routine disclosures of your PHI by us within the past 6 years (excluding disclosures for medical, payment, or operational purposes, and disclosures authorized by you).
You may request us to restrict the use or disclosure of your PHI (e.g., restricting the sharing of certain information with family members). We have the right to decide whether to approve the request, but if approved, we will abide by the relevant restrictions (except in cases where disclosure is required by law).
You may request us to communicate with you about PHI through a designated method (e.g., a specific email address, phone number) or at a designated location. We will approve the request as long as it is reasonable.
You have the right to request a paper or electronic copy of this notice at any time without paying a fee.
• We will comply with the privacy practices described in this notice and reserve the right to modify it at any time. The revised notice will be published on our website and take effect from the date of publication.
• We will ensure the security of your PHI and prevent unauthorized use or disclosure.
• We will not use your PHI for purposes outside the scope permitted by HIPAA unless we obtain your written authorization; if you authorize the disclosure of PHI, you may revoke the authorization in writing at any time (the revocation will not affect disclosures made based on the authorization before the revocation).
If you believe your PHI privacy rights have been violated, you may file a complaint with our Privacy Officer or directly with the U.S. Department of Health and Human Services (HHS). We will not take discriminatory measures against you because of your complaint.
